The AISIO mechanism that blames the orchestrator for system failures. Follows the orchestrator's career for life — affects insurance, future employment, the right to hold any certification. Used against Nadia Cho when the system made decisions in her name. Implementation variance does not require the orchestrator to have reviewed the decision. The Act makes this harder but does not make it impossible.
